Required Annual Forms

• Dental Form (necessary for students in kindergarten, 3rd, and 7th grades)

• Health Assessment (required for students in Kindergarten, 3rd, and 6th grades)

• Immunization Requirements (WSP adheres to all Pennsylvania state regulations concerning immunizations required for attending school.)

• Textbook Loan Form (one per family, required for all students)

• Lead Testing Form (required for Kindergarten students ages 5 and older. Also found on the health assessment.)

 

Supplementary Forms

• Prescription Drug Permission (for students who take prescription medications to be administered during the school day)

• Self-Administered Rescue Medication Permission (for students who carry inhalers or EpiPens in their backpacks)

• Walk Home Permission Slip (for students in 1st through 8th grade who have permission to check themselves out of school and walk home.

• Nursery Financial Agreement (complete all highlighted areas)

• Morning Garden Transitional Agreement (complete all highlighted areas)

• Emergency Contact Form for Nursery and Extended Care Programs (each section must be entirely filled out)
